1st Part

  • You need to fill up the society name in the first blank row.

  • Then you need to write your name along with the joint owner's name if any.

  • Next, type your society name and then the detailed address of your flat.

2nd Part

  • Then you have to enter share certificate details only if society has provided you with it, otherwise leave it bank.

3rd Part

  • Write down the details like flat address, area, wing number, etc.

4th Part

  • Here, you have to mention details of the person you want to nominate like name, address, and relation.

  • You need to mention the percentage you want to nominate the person for.

  • Mention the date of birth if a person is a minor.

5th Part

6th Part
