How to fill nomination form?

Hello buddy! I recently performed the DA 1 nomination form fill up. The process is quite simple. I will help you with the method for Canara Bank through this answer. I’ll also attach the link to the form for you.

How to fill form DA 1

1. In the first space denoted as I/We, write the name of the account holder, this means you need to write the name of the person in whose name the bank account has to be entered. Also, mention the complete address.

2. Write the name and complete address of the bank and its branch.

3. In the table, write the details of the “Deposit” on the left-hand side. Here you need to fill in the name, nature, and distinguishing No., and any additional information. On the right-hand side, register the details of the “Nominee”, such as the address, age, if the nominee is a minor, write the name of the appointed person.and their address alongside their age.

4. Fill in the place and date in the space provided below the table.

5. The depositor has to sign in the space provided on the right.

6. Next, write the name and address of the two witnesses. They have to sign as well.

7. Lastly, write the date of the day when the nomination gets accepted.

How to fill out the nomination form DA1 of the Indian Bank?

1) Branch: In this we have to write the name of our bank branch, meaning the area in which the bank is located.

For example, if someone's bank is in Camp, Pune, then his branch is Camp.

2) I/We: In this, we have to enter the name of the account holder, which means, in whose name the bank account has to be entered and the full address has to be entered.

3) Indian Bank: We have to write the name of our bank branch.

4) Nominee Particulars

5) Name and Address: The name and full address of the one you want to make a nominee have to be entered.

6) Relationship with depositor if any: What is your relationship with the account holder?

7) Age: The nominee's age has to be entered.

8) If minor, date of birth: If the nominee's age is less than 18 years, his full date of birth has to be entered.

9) Deposit Particulars

10) Type / Scheme: Enter the type of account in this, and what type of account is there.

For example, a savings account, FD account, etc.

11) Account / Receipt No: You need to enter the account number.

12) Date: You need to write the date 

13) Amount: If any 

14) Amount: If there is a fixed amount then enter.

15) Maturity Date: If there is any maturity date then enter.

16) I/We appoint Mr/Ms: You can enter the name and address of any member of the nominee's family. If Nominee's age is less than 18 years on the date on which you will submit this form to the bank branch. If for some reason something happens to the nominee, then all the money will go to the nominee's family. But if the nominee's age is above 18 years, then you can leave this option as it is.

17) Signature / Thumb impression of the Depositors: Here the account holder has to be signed, the way you sign in the bank. If someone puts a thumb impression, then he has to put his thumb.

18) Place: Enter the name of the place where you live.

19) Date: Enter today's date.

20) Witnesses for Thumb Impression

If the account holder puts a thumb, then two people will have to name in witness for that.

21) Signature: Signature of the witnesses.

22) Name: Enter the name of the Witnesses.

23) Address: Enter the address of the Witnesses.

24) Place: Enter the name of the place where you live.

25) Date: Enter today's date,

This is how the Indian bank nomination form will be filled. What you have to do next is to take your bank account passbook, aadhar card original and photocopy, along with the PAN card and photocopy of the person you are going to make nominee. You will have to go to your bank branch and here you will have to give these documents to the bank employee. After that, the receipt will be given to you from the receiving bank, which you should keep with you. So now you know how to fill nomination form DA1 Indian Bank.
