One of the key steps in applying for a reissue is obtaining an affidavit for a lost passport, which serves as a formal declaration detailing how the passport was lost and confirming that it is not in your possession. But how do I get an affidavit for a lost passport? 

  1. The process begins by drafting the affidavit on non-judicial stamp paper (generally Rs. 10 or Rs. 20 denomination) with accurate details about the incident. This includes your full name, passport number (if known), date and place of loss, circumstances under which it was lost, and a statement confirming that it has not been used for any illegal activity.

  2. This brings us to the next essential step: lodging a First Information Report (FIR) at the local police station and obtaining a copy of the FIR or a police report. This document, along with the affidavit, must be submitted when applying for a duplicate passport at the Passport Seva Kendra.

  3. Once the affidavit is drafted, visit a Notary Public or First-Class Magistrate with valid identity documents such as Aadhaar, PAN card, or voter ID.

  4. The notary will verify your identity, witness your signature, and notarise the affidavit with an official seal.
