Prepare a draft affidavit stating how and where the passport was lost. You can use Annexure 'F' as a template, which is available on the Passport Seva website.
The affidavit must be notarised by a First Class Judicial Magistrate, Executive Magistrate, or a Notary Public.
Include the notarized affidavit when you apply for a re-issue of your passport. You'll also need to provide other documents such as proof of address, proof of date of birth, and any other required documents.
