  • Then prepare the affidavit with your full name, father’s name, address, PF account number, UAN, employer details, employment period, and the reason for submitting the indemnity bond.

  • The affidavit must clearly declare that you are the genuine PF account holder and that you will take responsibility if any future dispute or false claim arises regarding the PF withdrawal.

  • After drafting the affidavit, attach supporting documents such as Aadhaar card, PAN card, bank passbook copy, PF statement, and resignation or relieving documents if available.

  • Sign the affidavit in front of a Notary Public or First-Class Magistrate because notarization is mandatory in most PF indemnity cases. 

  • Many EPFO offices also ask applicants to attach Form 19, Form 10C, a cancelled cheque, and employer verification along with the affidavit.
