Click "Card Related Service Request" on the right side of the home page, beneath the handy links
When you click that link, you'll be given three alternatives, one of which is to
1) Add a new family member
2) Remove a member of the family
3) Address change
Select the one that is most appropriate for the task at hand. My cousin selected the second option and followed the instructions to remove his name from the card.
