You must provide your full name if you are a claimant.
You must provide an accurate home address.
You must provide the date of birth and the name of your father.
Your complete address must be provided in addition to your mobile number.
Your transaction information must be provided (including the amount).
You must specify the location where you submitted your tax returns.
You need to provide your Aadhaar details.
If you have not yet received your PAN, you should provide the acknowledgement number and the date you registered for it.
