So I can say from my experience that the count will include any door that is in contact with the ceiling wall. There are some Vastu guidelines to follow when counting the numbers, neither the main gate nor the doors outside the house will be counted. Also, if there are two flanged doors in your house, they must be regarded as one door.

Before answering this, I think I should share with you what doors and windows should we count. As per my Vastu expert, we should always count the opening towards the balcony, and main entries. The same goes for windows. We should also count the doors and windows floor wise.
There should be 2,4,6,8, (even) no of doors in house as per Vastu. My Vastu expert told me we should always stick to even numbers except for 10. The main doors should have no obstructions. Any plants, trees, staircases, or poles, can impact the positive energy of the house. And it's better to avoid hanging any pictures or depictions of God just outside the door.
Windows should be constructed in rooms to ensure that every room is well ventilated. So, doors and windows should be placed in such a way that proper ventilation is possible. To invite the positive energies we should always keep the windows and doors open at least once during the day.
