As far as I know, any corrections in Form D land issues can be made after submitting the application for correction along with relevant documents to the sub-registrar/ tehsil officer. You will have to provide details such as:
Personal details: your name, father's/husband's name, gender, age, occupation and Aadhaar number
Property details: District, Village, Survey Number, land ID, form D details, Passbook and registration number, etc.
