For Pension person:You have to download the form from the official website and get it verified by the respective authorized person. After you verify the form, attach your photograph, fill it with all the essential details and then submit it to the Additional Director or Joint Director’s (AD/JD) office along with all the required documents.
For serving employees:The individuals who are working in any organization or company need to fill out the form by downloading it from the official website. Then they have to attach a photograph and all the required documents and then submit them to the place where they work. The application will be provided ahead to the Additional Director of the CGHS for issuing the CGHS plastic card. You can use that printed form to avail facilities at CGHS health centres.
Eligibility to apply for CGHS scheme:
1) Employees of the central government, as well as retirees and their families, are eligible for the scheme if they live in CGHS-covered areas.
2) They should be Freedom Fighters, Ex-Governors, Lt. Governors, retired Judges of High Court, Railway Board Employees, Current and Ex-members of Parliament and Post and Telegraph Department employees.
3) All these persons are eligible for the CGHS scheme.
