Using UAN:
- Visit the EPFO portal
- https://unifiedportal-mem.epfindia.gov.in/memberinterface/
- Login with your UAN ID and password
- Enter your Captcha Code
- Click on Online Services
- Select Form 31, 19, 10C & 10D
- Enter your bank account number
- Click on Proceed for Online Claim
- Select reason for withdrawal
- Provide all details and your claim application will be submitted
