I visited the bank to learn about the home loan DSA registration process online but was asked to download the form from their official website and visit the bank with the filled form and necessary documents.

What Documents are Required for DSA Registration?
Each bank asks for a set of documents for DSA registration but the mandatory documents required for it are:
Identity proof like Aadhar Card, Voter ID card, Driving licence, or others
Address proof like Aadhar Card, Voter ID card, Bank Statement or others
Proof of income
Passport size photographs
