The first thing that you need to check is if the specimen signature card for the employer PF form is meant for the EPF organisation. It will be mentioned in the title of the form.
Now you have to fill in the name of your company and the address of the company or establishment in the specimen signature card PF form.
Then you will have to mention the name/names of the employer/employers and their designation.
You may find multiple columns for multiple employers' names and signatures. If your organisation is a partnership firm, then you may have to write the name of all partners, and their designations and get their signatures.
Then you will be able to see a few columns under the specimen signature. There you will have to get the signature of your employer/employers.
