Gazette Publication is required for govt. employees and optional for others. However, if you have to update your name on several id cards and certificates, they can request you to submit a Gazette copy. Let me tell you how to change name in Gazette.
How to Make Gazette for Name Change
If you want to publish your name change in Gazette, then you will have to visit the office of the Controller of Publication (Department of Publication).
Documents needed to publish your name in the State Gazette:
- An affidavit signed by the applicant and attested by Notary/Judicial Magistrate.
- Original and copies of advertisements published in the newspapers.
- Computer typed Prescribed proforma (not handwritten) signed by the applicant as well as two witnesses.
- A C.D. that includes the soft copy of the application in MS Word format (write your name in the place of the signature)
- A duly signed certificate declaring that the contents included in both hard copy and soft copy are similar.
- Two self-attested passport-size photographs.
- A self-attested copy of an ID proof.
- A request letter as well as the required fee (approx. Rs. 700-900) as per the Authority.
