Documents needed to publish your name in the State Gazette:An affidavit signed by the applicant and attested by Notary/Judicial Magistrate.
Original and copies of advertisements published in the newspapers.
Computer typed Prescribed proforma (not handwritten) signed by the applicant as well as two witnesses.
A C.D. that includes the soft copy of the application in MS Word format (write your name in the place of the signature)
A duly signed certificate declaring that the contents included in both hard copy and soft copy are similar.
Two self-attested passport-size photographs.
A self-attested copy of an ID proof.
A request letter as well as the required fee (approx. Rs. 700-900) as per the Authority.
The money can be paid either in cash or by a postal order/demand draft drawn in favor of the ‘Controller of Publication, Department of Publication, Civil Lines, Delhi-54’.
